Restorating Endodontically Treated Mandibular Molars With Ceramic Onlays

Abstract

Background: A ceramic onlay is a form of partial-coverage restoration with excellent esthetics which is used as an alternative of a full-coverage crown. Endodontically treated teeth are more brittle and easy to fracture so they need adequate restoration after endodontic treatment like onlays that can be designed with resistance. Purpose: The aim of this article is to explain the procedure of endodontic restoration to retain tooth structure and restoration of function. Case(s): A 29-year-old female came to the Department of Conservative Dentistry, Airlangga University with the chief complaint of spontaneous pain in the right lower back tooth for 1 week. Clinical examination revealed a deep carious lesion in the mandibular right first molar. Radiographic examination revealed radiolucency involving the pulp. Case Management: root canal treatment was performed. Ceramic onlay tooth preparation was performed. 1.5-2.0 mm of occlusal clearance was performed to prevent fracture. A chamfer was placed on the facial, lingual, and proximal surfaces with supragingival margin. Preparations were recorded with heavy and light body and then sent to the laboratory. As esthetics is one of the prime concerns for the patient, IPS emax was selected for the fabrication of the ceramic restoration. Discussion: Conservation, preservation, and reinforcement of tooth structure of an endodontically treated tooth can also be achieved by partial-coverage restoration rather than full-coverage restoration. With the esthetic restoration and improved adhesive techniques, esthetic onlays like ceramic onlays can be used to preserve the maximum amount of health tooth structure. Ceramic onlays satisfy both the functional and esthetic demands of the patient.
