Apexification of an Immature Permanent Central Incisor with Mineral Trioxide Aggregate

Abstract

Traumatic injuries during the root formation stage may lead to cessation of root maturation which results in a wide apical foramen. Mineral trioxide aggregate (MTA) was introduced as a root filling material due its biocompatibility and ability to seal the root canal system. Purpose: To demonstrate apexification with the mineral trioxide aggregate apical plug technique. Case: A 17-year-old female patient with a non-contributory medical history was referred for endodontic treatment of the maxillary right central incisor. Her dental history revealed that the patient suffered a trauma at the age of 8 years old. The chief complaints were an unesthetic appearance of the tooth due to dark gray discoloration and discomfort during masticatory function. The radiographic appearance showed a wide apical foramen with a periapical lesion. Case management: The working length was measured with an apex locator followed by debridement using a K-file. The canal was irrigated by sodium hypochlorite and calcium hydroxide paste was used as an intracanal dressing for one week. At the second appointment, a MTA apical plug technique was performed to create a barrier at the wide apical foramen, and then the remaining root canal space was obturated with thermoplastic gutta percha. After 1 week, the canal was prepared for fiber post insertion, and finally the tooth was restored with a porcelain crown. Conclusion: A necrotic immature permanent central incisor can be successfully treated with a MTA apical plug technique.
