Rehabilitation Management for Traumatic SCI During Pregnancy: A Case Report

Abstract

Traumatic spinal cord injury during pregnancy presents great challenges to both mother and fetus. A choice of management including rehabilitation intervention for spine fracture, orthosis and ambulatory devices needs to be made on a case-by-case basis with careful consultation between physiatrist, orthopedic surgeons, and obstetricians. We present a case of spinal cord injury during pregnancy, resulting from a motorcycle accident. A 33-year-old female presented with gravida 3, para 2, in the first trimester diagnosed with L2 AIS-B complete paraplegia. She also had multiple open fractures of the right upper and lower limbs. She had difficulties in ambulation, urinating and fecal retention, decrease of respiratory capacity and mild depression. Our rehabilitation goals were to avoid complications of prolonged immobilization, and prevent further damage of the vertebrae and extremities. Our programs include psychological support, early mobilization with a customized thoraco-lumbosacral brace and wheelchair, ROM exercise, breathing and chest expansion exercises, endurance exercises, core stabilization, and bowel and bladder training. Using a comprehensive rehabilitation program, the patient showed improvements in ambulation, the level of Asia Impairment Scale from B to C, cardiorespiration function and the depression scale as well
