Comprehensive Program of Degenerative Scoliosis in Reducing Pain

Abstract

Degenerative scoliosis needs to be distinguished from adult scoliosis as it is a result of progressive degeneration of structural spinal elements leading to spinal column malalignment. With the increasing aging population in Indonesia and increased attention to improving the quality of life of the elderly, degenerative scoliosis should be considered as a cause of significant pain and disability. Identified causes of pain include asymmetric degeneration of the discs, osteoporosis, and vertebral body compression fractures which require selection of appropriate multimodal intervention including exercise, modalities and orthoses to reduce pain and the results are variable. We evaluated the effect of our comprehensive program in geriatric patients with degenerative scoliosis for reducing pain (visual analog scale/VAS as tool). Nine geriatric patients (age range from 60 to 78 years old) who met our inclusion criteria were included in this study. Our comprehensive program consisted of scoliosis-specific exercises, modalities and dynamic traction (vertetrac and/ cervico) for 5 days consecutively. VAS before and after programs were assessed before and after programs, with statistical analysis using paired t-test. The VAS before programs had a wide range from 5 to 10 (mean ± SD: 6.67 ± 1.50) while VAS after programs had reduced (mean ± SD: 1.89 ± 2.03). Age and total sessions was analyzed as predictor factors using regression analysis. VAS reduced significantly (p=0.000) after a minimum of 5 consecutive days of the program. Age and total sessions were not significant as factors influencing treatment outcome (p=0.839 and 0.054). A comprehensive program consisting of scoliosis-specific exercises, modalities and dynamic traction for degenerative scoliosis has been proven to reduce the pain of geriatric patients
