A Dengue Haemorrhagic Fever Patient with Expanded Syndrome of Acute Hepatitis

Abstract

This is a report a male patient aged 29 years old with a chief complaint of fever for 5 days and other complaints such as chills, malaise, headache, joint pain, and jaundice. The diagnosis of the patient was DHF gr II with expanded dengue syndrome of acute hepatitis. The evidence was 8000/mm3 thrombocytopenia, hemoconcentration, NS1 positive, IgG and IgM dengue positive, elevated aspartate transaminase and alanine transaminase > 10 times normal, and hyperbilirubinemia, hepatomegaly, and viral hepatitis A, B, C serology were all negative. The probability of secondary dengue infection was indicated by positive IgM and IgG in this patient. These are high-risk factors contributing to the degree of severity of the disease. A liver complication in DHF is not uncommon; however, the most common manifestation is mild to moderate. A severe liver complication is rare and found in less than 10% of all dengue patients. It usually manifests in acute hepatitis with an increase in transaminases 10 times more than normal, jaundice, hyperbilirubinemia, hepatomegaly, and fatal acute liver failure. The expanded syndrome is a new entity termed by WHO-SEARO guidelines in 2011 as including various atypical organ manifestations such as neurological, hepatic, renal and other isolated organ involvement that has been reported around the world. The reported patient experienced a worsening condition that is a shock (DHF gr III); however, recovery was possible with the proper and adequate intravenous fluid rehydration. The patient was finally discharged on the 7th day after admission with an improved general condition, platelets of > 50,000/mm3, and improvement of the liver’s condition
