Treatment of Internal Bleaching after Apexification

Abstract

Background: One of the most common causes of pulp necrosis is trauma. Pulp necrosis of an immature permanent tooth can cause apex closure to be inhibited that is sometimes followed by discoloration. Purpose: To provide clinical and radiographic information about open apical treatment with apexification using MTA and tooth discoloration with internal bleaching using the walking bleach technique. Case: A female patient, 21 years old, came with dental complaints of tooth discoloration due to a trauma, which happened 8 years ago. The tooth had never been treated. On clinical examination, tooth 22 was found to have a gray discoloration. The condition of the crown was intact. The tooth was in normal condition on palpation and percussion. Radiographic examination showed an apical foramen that was not completely closed. Case management: root canal treatment was performed on tooth 22, root canal was irrigated using saline solution and a calcium hydroxide dressing was applied for 1 week. At the second appointment, apical closure was performed using MTA and then obturation was performed using thermoplastic gutta percha. At the third appointment, internal bleaching was performed with the walking bleach technique using 35% hydrogen peroxide. The patient was recalled for subsequent visits at the 3rd and 6th months after completion of the treatment. The tooth functioned optimally, and a radiographic image did not show any pathology. Conclusion: Necrotic immature permanent teeth can be treated successfully with the MTA apical plug technique, and tooth discoloration can be treated effectively with the internal bleaching technique.
