Proper Nonsurgical Management Of Mandibular Comminuted Symphysis And Condylar Facture: A Case Report

Abstract
Background: The dilemma related to nonsurgical management of mandibular comminuted symphysis and condylar fractures is the duration of intermaxillary fixation due to adequate time for bone callus formation and the risk of TMJ ankylosing. The prognosis and outcome of this type of fracture management has to be determined due to the variety of treatments. Purpose: Comparing the indications, treatment planning and the prognosis of mandibular comminuted symphysis and condylar fracture managements. Case: A 23-year-old patient reported to have been the victim of a motorcycle accident 6 days beforehand and complained of difficulty chewing and closing the mouth. There was evidence of closed fracture with laceration at the right anterior part of the mandible. Intraoral examination revealed bone discontinuity in the symphysis region and malocclusion. Case Management: The patient was treated with an archbar for 6 weeks: 4 weeks for intermaxillary fixation and 2 weeks of limited mandibular movement using elastic guidance. Over the period of the evaluation, the patient ate soft meals. Conclusion: Comprehensive treatment planning depends on the extent of injury, the level of fracture, the degree of displacement and dislocation, the dental malocclusion and mandibular dysfunction, general state, and the willingness of the patient to be treated. The mandibular comminuted symphysis and nondisplaced condylar fractures are best treated nonsurgically by closed reduction and intermaxillary fixation.
