The Role of Vanilloid and Cannabinoid Receptors in Taste and Pain Perception in Burning Mouth Syndrome

Abstract

Burning mouth syndrome (BMS) refers to chronic and idiopathic pain in the oral cavity. Common clinical symptoms include taste perception disturbance and persistent dysgeusia, which present along with chronic pain. This article aims to review the role of vanilloid and cannabinoid receptors in taste perception and pain in BMS patients. The peripheral gustatory system not only comprise taste buds, but also the peripheral nerves innervating them. In patients with BMS, any changes in the system may affect the perception of taste and cause pain. Increasing number and distribution of vanilloid receptor TRPV-1 and cannabinoid receptor CB2, accompanied by decreasing cannabinoid receptor CB1, contribute to pain and taste perception changes. TRPV-1 is specifically sensitized by capcaisin or spiciness. Capcaisin stimulates trigeminal nerves to release prostaglandin which trigger free nerve endings to capture spicy taste perceived as pain. Increasing distribution of cannabinoid CB2 receptors on papilla foliate may cause threshold elevation of sucrose and a sour taste. In conclusion, changing both the number and distribution of vanilloid receptor TRPV-1 and cannabinoid receptor CB1 and CB2 in BMS may trigger abnormal activation of these receptors, resulting in pain and threshold change of sour taste, sweet taste and bitter taste.
