Chronic Oral Ulcer due to Secondary Infection of Proteus Vulgaris

Abstract

Chronic oral ulcers (COUs) can occur because of secondary bacterial infection. The infection impairs the treatment and the healing process. The kind of bacteria which induce the infection could originally come from normal flora or the environment. This paper reports the management of COU in a 49-year-old man. A 49-year-old man complained of a painful persistent ulcer on his left buccal cheek for 5 months. Lesion preceded because of toothbrushing trauma. The lesion never healed and became wider. There was no prodromal phase and no other lesion found in the oral nor cutaneous region. Several tests were ordered and the patient was treated symptomatically using anti-inflammatory mouthwash and vitamin C. A complete blood count (CBC) showed elevated ESR, and microbiology examination was positive for Proteus vulgaris culture that is sensitive to floroquinolone. Liver function, renal function and blood glucose level were normal. The patient was then treated with ciprofloxacine orally, anti-inflammatory and antiseptic mouthwash, and vitamin C. Recovery was achieved within 6 months’ follow up. Chronic ulcers are commonly caused by continous mechanical trauma to the oral mucosa. Secondary infection of bacteria causes long-lasting ulcers. There are some factors that expose people to the risk of secondary infection by Proteus vulgaris including 1) being immuncompromised, 2) taking a long-term antibiotic regimen, 3) experience of urinary tract infection, 4) living in an inappropriate environment with poor sanitation. Proteus vulgaris has an extra cytoplasmic outer membrane which contains a lipid bilayer, lipoprotein, and polysaccaharides, and produces protease that inhibits cell regeneration and apoptosis. Proteus vulgaris is least resistant to ciprofloxacine and cefotaxime, but when it is introduced to these drugs, higher doses than normal should be used.
