Esthetic Correction of a Gummy Smile by a Conventional Gingivectomy: A Case Report

Abstract
A gummy smile is due to excessive gingival display while smiling. It is caused by a skeletal deformity that involves vertical maxillary excess, a short upper lip, insufficient clinical crown length, or a combination of the above. A-32-year-old male patient visited the Periodontic Clinic of the Dental Hospital Airlangga University with the chief complaint of excessive and dispropotionate gum display while smiling. The patient wanted the excessive gum displayed to be corrected. On examination, teeth 13-23 (FDI notation) showed excessive gingiva. Radiographic examination found that the distance between the Cemento Enamel Junction (CEJ) and alveolar bone crest was more than 1.5 mm. The case was diagnosed as type I Altered Passive Eruption (APE). A gingivectomy using a conventional method was planned for this case on teeth 13-23. Gummy smile correction by a gingivectomy aims to increase the crown length for esthetic purposes. The surgical procedure is aimed at re-establishing the biological width and exposing more tooth structure. A gingivectomy using a conventional method gives satisfactory results in treating a gummy smile in type I Altered Passive Eruption (APE) cases.
