Management of Oral Lichen Planus Due to Stress In Post-Menopausal Women

Abstract

Introduction: Lichen planus is a common chronic inflammatory disease in the skin and mucosa. Usually suffered by woman in middle age more affected than men. The clinical characteristics of oral lichen planus (OLP) are white reticular papules mediated by the cellular immune system characterized by cytotoxic T cell responses to basal cell keratiocytes with psychic stress as trigger factors. The diagnosis of OLP based on typical clinical features is supported by histopathological examination. Purpose: to report the management of stress-induced OLP cases in postmenopausal women. Case: 45-year-old female patient came with painful complaints on under the right and left gums since 5 months ago, missing relapse. When the patient's history is admitted in a stressful state. Intra oral clinical examination of gingiva and buccal mucosa is white papule (wickham's striae). Management: patients treated with benzocaine gel, topical corticosteroids, systemic corticosteroids, avoiding spicy and spicy foods, and DASS 42. Then the patient is referred for full blood examination, current time glucose examination, SGOT and SGPT, and BUN and Kreatinin, and Refer to a psychiatrist. Conclusion: moderate depression, anxiety and stress in postmenpausal patient considered a precipitating factor cause altered immune system that triggered autoimmune disease. Treatment given by managing stress as well as symptomatic therapy with systemic and topical corticoseroid and maintain oral hygiene.
