Maternal Anthropometrics as a Predictor of Preeclampsia Risk Factor

Abstract: Preeclampsia (PE) is still a major cause of maternal and fetal mortality and morbidity in both well-resourced and under-resourced settings, while the etiology of this disease remains uncertain. Both overweight and obesity have been shown to be strong predispositions of preeclampsia that can be assessed by several methods. The clinical relevance of those different anthropometric methods with preeclampsia is still being debated. The objective of this study was to examine the association between maternal anthropometrics (body mass index (BMI) pre-pregnancy, BMI in the third trimester, and upper arm circumference (UAC)) and preeclampsia in order to determine which anthropometric method is the best predictor of preeclampsia. A hospital-based case-control study was conducted between February and September 2017 at dr. M. Soewandhie Surabaya state hospital and Universitas Airlangga teaching hospital. Subjects were 51 preeclampsia mothers and 51 control subjects. Data were collected by interviewing subjects and accessing medical records. Subjects were categorized into two groups: an underweight/normoweight group and an overweight/obese group. The odds ratio (OR) and 95% confidence interval (CI) were estimated using logistic regression. The results showed that only overweight/obesity determined by measuring BMI in the third trimester was a significant risk factor for preeclampsia (OR=3.762; 95% CI 1.472–9.616; p=0.008). This finding supports the view that weight gain that occurs in the third trimester is a major concern in relation to preeclampsia.
