Abstract
Patient safety is the prevention of errors and adverse effects to patients associated with health care. While health care has become more effective, medicines and treatments have also become more complex. Sidoarjo General Hospital has implemented patient safety standards. The reporting rate of patient safety incidents between 2012 and 2015 was 31.83%, which was still below the standard set by Standardizing Medication Error total of 40.3%. This study aimed to provide some recommendations to improve the Reporting of Patient Safety Incidents based on integrity and commitment. This research was a quantitative analysis with a cross-sectional study. It was carried out over 4 months and the sample was 33 teams. This study showed the influence of integrity and commitment to the team reporting incidents. The recommendations of this study are to increase the activity of the human resources to report incidents of patient safety and increase understanding of the type of incidents of patient safety which need to be reported to the Patient Safety Team and reduce errors in reporting incidents through a management information system. The implementation of Reporting of Patient Safety Incidents shows a good level of integrity and commitment from Sidoarjo General Hospital which is very good.
